失误 — shīwù

1. lapse; mistake; fault (also used as a verb)

   他在比赛中出现了一个失误，球队因此输了比赛。 tā zài bǐsài zhōng chūxiàn le yígè shīwù qiúduì yīncǐ shū le bǐsài — He made a mistake during the match, and the team lost because of it.

2. to make a mistake; to commit a fault (e.g. service fault in volleyball/tennis)

   发球时他失误了，裁判判为双误。 fāqiú shí tā shīwù le cáipàn pànwèi shuāng wù — He made a mistake on his serve and the referee called it a double fault.
